For Employed Individuals:

All employees working in the private or public sector
Steps:

1. Ask your HR to help you with your PhilHealth registration (usually done during onboarding).

2. Fill out the PhilHealth Member Registration Form (PMRF) – available at HR, PhilHealth branch, or download it here.

3. Submit a valid government ID (e.g., birth certificate, UMID, passport).

4. Your employer will submit the documents and process your membership.

5. Once registered, you’ll be issued a PhilHealth Identification Number (PIN).
Contributions:

Your employer will share the monthly contribution cost with you (deducted from payroll).
For Self-Employed / Voluntary Members

Freelancers, small business owners, farmers, tricycle drivers, or any individual earning without a formal employer.
Steps:

1. Visit the nearest PhilHealth office or use PhilHealth’s online registration.

2. Fill out the PMRF and tick “Self-Earning Individual” as your membership type.

3. Submit a valid ID and proof of income if available (e.g., affidavit of income).

4. Pay your initial contribution at PhilHealth, a bayad center, or online payment partners.

5. Receive your PhilHealth ID and Member Data Record (MDR).
Contributions:

Based on your declared income. Rates are updated yearly (as of 2025, it’s about 4% of monthly income, with a minimum and cap).
For Indigent Members

Low-income or unemployed individuals identified by DSWD or your LGU.
Steps:

1. Go to your local Barangay Health Center or LGU social welfare office.

2. Ask about being assessed for the PhilHealth Indigent Program (sponsored membership).

3. Submit documents like:
– Barangay Certificate of Indigency
– Valid ID (if available)

4. If eligible, the LGU or DSWD will endorse and register you with PhilHealth.
