How to Get a PWD ID Card in the Philippines

If you or someone you know has a permanent disability, you can apply for a PWD ID to enjoy various discounts and privileges under Republic Act 9442, also known as the Magna Carta for Persons with Disabilities.

While every city or town might have its own process for obtaining a PWD card, here are the general steps on how to apply for a PWD ID:

Who Can Get a PWD ID?

You may qualify for a PWD ID if you have any of the following:

  1. Psychosocial or mental disabilities
  2. Chronic illnesses (e.g. dialysis, severe heart disorders, cancer)
  3. Learning disabilities
  4. Visual or hearing impairments
  5. Speech or orthopedic problems
  6. Other similar conditions that limit your ability to do daily activities

The PWD ID gives you access to benefits like:

  • 20% discount on medicines, transport, restaurants, and more
  • Priority in lines and services
  • Other support services under Philippine law

Where to Get a PWD ID Application Form

You can get the PWD Registration Form (PWD-RF) from:

  • Your Mayor’s Office
  • Barangay Captain’s Office
  • DSWD or NCDA regional offices
  • Participating NGOs with agreements with the Department of Health
  • For some cities, like Quezon City, offer online registration through their e-services platforms. 

What You Need to Apply

Prepare the following:

  1. Two 1×1 ID photos with your name and signature or thumbmark at the back
  2. One (1) valid ID
  3. Proof of disability, such as:
  4. Medical Certificate (for most disabilities)
  5. School assessment (for learning disabilities)
  6. Certificate of Disability (from NGO or workplace head, if applicable)

How to Apply for a PWD ID

You can apply in two ways:

Manual Application

  1. Get the form from your barangay, mayor’s office, or DSWD
  2. Fill it out completely
  3. Attach your photo and documents
  4. Submit it to your Mayor or Barangay Captain

What Happens Next?

The Mayor or Barangay Captain will:

  1. Review and verify your documents
  2. Assign a unique PWD ID number
  3. Fill out your ID card and give it to you
  4. Submit your data to the Department of Health’s PWD Registry System

Validity and Renewal

  • Your PWD ID is valid for 3 years
  • The first ID is free
  • Lost or expired IDs can be replaced for a small fee

Important Reminder

The PWD ID is an official government-issued ID. Using it illegally or faking disability status is a crime and will be penalized.
