How to get medical assistance from DSWD

The Department of Social Welfare and Development (DSWD) offers medical assistance under the Assistance to Individuals in Crisis Situation (AICS) program.

If you or a loved one is struggling with hospital bills, medicine, or lab procedures, here’s a simple guide on how to apply.

Basic Requirements

You need to prepare:

  • Valid ID (of the patient or representative)
  • Authorization Letter (if applying on behalf of someone else)

For Hospital Bills

Prepare the following:

  • Medical Certificate / Clinical Abstract / Discharge Summary / Alagang Pinoy Tagubilin Form
    ▸ Must show diagnosis, patient’s full name, physician’s license number and signature
    ▸ Must be issued within the last 3 months
    ▸ Original or certified true copy
  • Hospital Bill or Statement of Account with:
    ▸ Outstanding balance, name and signature of billing clerk
    ▸ OR Certificate of Balance + Promissory Note signed by the credit/billing officer
  • Social Case Study Report or Case Summary (from a hospital social worker)

For Medicine or Assistive Devices (e.g., wheelchair, hearing aid)

Prepare the following:

  • Medical Certificate / Clinical Abstract / Discharge Summary / Alagang Pinoy Tagubilin Form
    ▸ Must show diagnosis, patient’s full name, physician’s license number and signature
    ▸ Must be issued within the last 3 months
    ▸ Original or certified true copy
  • Prescription
    ▸ Includes date, name of patient, doctor’s license number and signature
  • If request is more than ₱10,000, include:
    ▸ Quotation of Medicine/Device
    ▸ Social Case Study Report or Case Summary

For Laboratory, Medical Procedures or Operations

Prepare the following:

  • Medical Certificate / Abstract / Discharge Summary / Alagang Pinoy Tagubilin Form
    ▸ Must show diagnosis, patient’s full name, physician’s license number and signature
    ▸ Must be issued within the last 3 months
    ▸ Original or certified true copy
  • Laboratory Request / Doctor’s Order / Protocol
    ▸ With doctor’s name, license number, and signature
  • If request is over ₱10,000, include:
    ▸ Quotation of laboratory or medical procedure
    ▸ Social Case Study Report or Case Summary

Important Reminders

  • All documents should be issued within the last 3 months.
  • Assistance is free of charge and handled by DSWD’s Crisis Intervention Division.
  • Final approval depends on DSWD’s assessment and budget availability.

Visit the nearest DSWD Field or Satellite Office or inquire through your local social welfare office. Look for the Crisis Intervention Unit.
